The Palliative Aged Care Consultancy Service (PACCS) offers an evidence-based model of clinical nurse consultant-led palliative care needs rounds for aged care residents.
Our knowledge exchange, upskilling and education programs aim to increase the confidence and capacity of aged care staff in caring for their residents.
The PACCS Clinical Nurse Consultant visiting the RACF (Residential Aged Care Facility) is not a separate service provider but works as part of the facility’s care team, working collaboratively with the staff to assess, plan and manage a coordinated approach to each resident’s care.
Clinical Nurse consultants also provide education both in formal settings and informal settings within Aged Care Facilities.
